For patients looking for a rhinoplasty (nose job) but who don’t want to go under the knife, a new non-surgical nose-job may be the answer.

An anaesthetic cream is used on the nose. Injections of cosmetic filler are then injected around the nose to change it’s shape. The whole process takes around 10 minutes and can therefore be carried out during a lunch hour.

The advantages of the non-surgical nose job are clear – the treatments costs around a tenth of an actual rhinoplasty and doesn’t involve the typical six week recovery period of surgery. The side effects of the procedure are also far less severe than rhinoplasty surgery.

Non-surgical nose jobs can be used to help straighten out the nose’s appearance, help make the tip turn up instead of down, correct any asymmetry in the nose, and help smooth any bumps. Nasal dermal fillers can also help to soften the overall appearance of the nose.

However the non-surgical nose job is not suitable for people who want to make their nose smaller as it can only change the shape and not the size. The process is also not permanent. Dermal fillers are made from materials which are eliminated by the body’s lymphatic system and kidneys over time, therefore top-up fillers will need to be applied every year or so in order to maintain the new shape of the nose.

The non-surgical nose job is not a new procedure. Originating in Brazil around seven years ago, many countries are now offering the technique. However some cosmetic surgeons have expressed concern over the non-surgical nose job as they say that long-term studies of dermal fillers in the nasal area have yet to be carried out so it is difficult to determine if any long-term damage may occur.

Repeated dermal filler applications can also cause scar tissue to form which can affect the shape of the nose and leave the patient with a lumpier nose than when they originally started. However given that up to 18 percent of rhinoplasty patients will see side effects such as excessive bleeding, loosening cartilage in the nose and scar formation, many patients will see the nasal dermal fillers as a good alternative to cosmetic surgery.

Over the past ten years we have seen the introduction of many new and exciting ways of improving looks without having to subject ourselves to the surgeon’s scalpel. Clinics have popped up offering to fill wrinkles with new injections, smooth frown lines with muscle relaxing toxins, enhance lips with just a few shots of Restylane and, more recently, perform the feat of the so-called ‘non-surgical nose job’.

So what exactly is the non-surgical nose job?

The non-surgical nose job is a technique where dermal fillers are injected into the nose to fill out and/or smooth certain contours. For example, if the profile of your nose appears undulated, filler may be injected above and below the area which may smooth the appearance of the nose. Alternatively, if the tip of the nose sags and lacks projection, a small amount of filler may lift the tip into a more attractive position.

Are patients considering the non-surgical treatment being told the full story?

Criteria for non-surgical rhinoplasty are fairly specific, and it is unlikely the procedure will be beneficial for the majority of people wishing to undertake such surgery. It is improbable that the results will be as dramatic as they would be from a conventional rhinoplasty and, should the patient ever decide to have full surgery, the previous dermal filler injections may leave fibrous tissue behind that could impede the surgeon’s ability to deliver long term results. There is also the possibility of the other complications related to dermal fillers including allergic reaction, infection, granulomas and in rare cases skin necrosis.

Does this mean the non-surgical nose job is a bad idea?

No, not always – it just has very specific applications, and patients should be absolutely clear that the procedure is the right course for them. If you feel that a doctor or nurse is trying to ‘sell’ the procedure to you without explaining the possible long term implications then you should, as always, walk away.

Rhinoplasty – otherwise known as a ‘nose job‘ is one of the most common cosmetic surgery procedures. People undergo nose surgery for a number of reasons, the most common being:
- To reduce or increase the size of the nose itself
- To change the shape of the tip of the nose
- Narrowing the opening of the nostrils
- To change the angle between the hose and the upper lip
- To help relive some breathing problems

Am I suitable for a nose job?
Most surgeons will not operate on people under 16 if it is ‘elective’ surgery because they want to allow the nasal bone growth to complete before intefering with the nose. For those with medical purposes such as breathing problems or injurires, age is not really a factor.

Scars
After the surgery, small burst blood vessels might appear on the skin’s surface. These will be permanent. If the rhinoplasty is done using incisions inside the nostrils, no scarring will be visible. Some rhinoplasty surgery will leave small scars at the base of the nose, particularly for surgery to reduce the opening of the nostrils. Sometimes a second procedure will be necessary to correct minor problems.

The surgery itself
Rhinoplasty can be performed under either general anesthetic or local – it depends on both the patience preference and the severity of the surgery. Some people will be able to go home the same day. More complex procedures will require an overnight stay in hospital. The surgery should only take a maximum of 2-3 hours. Most surgery will be done via an incision inside of the nostrils.

Post-Surgery
You will be able to go back to work after a week of rest. Any activity that increases blood pressure such as exercise should be avoided for a month. Direct sunlight should be avoided to help the skin heal. If you wear glasses, these will have to be taped to your face for about 2 months to allow the nose to heal fully. Full results may not be apparent for up to a year.

What are the risks?
- Reactions to the medications
- Problems breathing
- Bleeding
- Infection of the incision sites
- Bruising
